1- I acknowledge that by registering my child for the youth conference, I am responsible for their safety and conduct throughout the event. While LDSHE volunteers will chaperone activities, they are not responsible for the direct supervision of individual participants.
2- I understand that I, or a designated guardian, must remain on-site at the conference venue for the duration of the conference and am accountable for my youth’s whereabouts at all times.
3 - By participating in the youth conference, I agree to abide by all conference rules and guidelines to ensure a safe and respectful environment for all attendees.
4- I understand that failure to comply with these requirements may result in removal from the conference without a refund.

I release LDSHE and its volunteers from any liability for incidents that may occur due to unsupervised youth or unforeseen circumstances.
